The primary role of the Community Health Organizer (CHO) is to: organize and mobilize elders around health related initiatives; engage in meaningful community dialogue about health issues; develop a community health and wellness calendar for residents to access a continuum of classes and workshops; and serve as a consistent point of contact, liaison, and advocate for health initiatives/ services.

CHO will also be responsible to provide case management and advocacy support; ensure strong participation in programs so that services have a positive impact on the health and wellness participants, their families and the community; build effective collaborations with other organizations, and engage in broader systems changes for the well-being of the community, based on the goals and outcomes established annually.

This is a regular, part-time (30 hours/week), non-exempt/hourly position located at Brian Coyle Center: 420 15th Avenue South, Minneapolis, MN 55454.
